    help with normalising risk assesment form

    Hello all,

    i have a risk assesment form that i need to normalise to 4th and BCNF, the problem is i havnt done anything like this for some time!

    i have scanned it in, and if some one would like to help me it would be greatly appreciated!

    i have scanned it in, ill need to send it to an email address. Its just 3 NF i require but im normally given a bit more information, ie tables and not a risk assesment invoice/form!


    Are you competent in normalization?
