It has been my experience, giving remote services and working with those that use that:

    1 - cost savings
3 - need for occasional extra help
    3 - after hours support
2 - don't want to hire a full time DBA
    1 - need specific expertise not in-house
5 - want to free in-house staff for more important work

There tends to be two types of database shops out there. Those that KNOW dba services are needed and actually hire someone and those that think a database can just run without any intervention or maintenance. This cause the second group to always look for a cheap resource to either patch ill-configured databases or just make sure they are runnign properly.

With most shops, this is really all they need. Those that really have and know their systems are critical will always have a dba on staff, don't typically allow them out of the basement, but will bring in dbas at time to augment work.

We are a small Remote DBA company and has been working for private and public sector since 1994. We have clients who work with us for more than 10 years.
I believe, the most important thing is experience and deep understanding of IT needs, practice and behavior. No on-site DBA could have so much experience and skills as a DBA company.
Cost saving is the second thing. Choosing a remote DBA companies save between 50% to 70%.
