Template for Adding

I am using a template for accessing a questionaire, with an "add" button
on it, when clicked (the add button), it takes me to a standard
listing of the fields, can you give me a sample or reference for using
a template for the add function. Thanks

Did you already look at all of the "Add" Examples at:
http://www.aspdb.com/Y2K ?

There are several there. They should help. What else do you need to know?
