DB help creating

I have a database that I need some assistance on. This db will keep track of volunter workers.
One table I will have
Person Name (First and last combined)- Text
Address- Text
DOB -Date
Tel Number- Text
Current Assignement Text
PPD Test -date
Orientation- Date


then I would like to have another table that will keep track of that persons hours by year for example
Jack Smith 1999 90000
Jack Smith 2000 100


I am not sure how to include this in another table and link it to the first table any help would be appreciated.
